"Crazy"Mental disorder, psychosis, schizophrenia, alienation, dementia ... depression, ... - many words, none fully satisfactory to describe profound disorders of the mind - those mentally ill can not access their rational mind - those who have similar deficits but retain the use of their reason often are said to suffer from "neurosis".  
Note in this image and in particular in the companion images how the artist convincingly conveys the idea that each of these men-patients is detached, distressed and focused on their "inner-self" - almost as if an orchestra or voices were whispering, shouting or threatening the "self" unrelentedly, persistently ... hopelessly ...  Each of the male patients depicted appear "detached" (alienated), unhappy (depressed), tired (distressed) ...
